4. Copia de su aplicación inicial (y/o última) de DACA
5. Dos fotos tamaño pasaporte (2x2)
6. Cheque o money order por $495*
7. Si ha sido arrestado, traiga los certificados de disposición de la corte de todos los arrestos

4. Copy of initial (and or all prior) DACA applications 
5. Two passport style photos (2 x 2)
6. Check or money order for $495*
7. If you have been detained or arrested, need to bring documention, such as certificates of minutes and final disposition for every incident

**Limited scholarships will be available to cover the $495 filing fee. Please bring copies of your 2016 Tax filing form or three pay stubs to qualify.

2. Legal permanent resident for at least 5 years (or 3 years if continuously married to U.S. Citizen)

Please bring: 

1. Legal permanent resident card
2. Social security card
3. ID/license 
4. List of complete addresses for the last 5 years
5. List of travel dates for the last 5 years
6. List of job history with addresses for the last 5 years
7. List of family information, including all children and spouses (current and former)
8. If you have been detained or arrested, need to bring documention, such as certificates of minutes and final disposition for every incident

Fee waiver eligibility:

1. Latest income tax return
2. Verification of benefits letter (if currently receiving SSI, food stamps, cash assistance)
